Health and Wellbeing Referrals

Live Life Aberdeenshire understands the pressures on the healthcare system and the information set out on these pages details the ways in which can support you as a healthcare professional.

We are happy to take referrals from healthcare professionals as our contribution to support the early intervention and prevention agenda.

You can refer your patient to our team if you feel there is a requirement for assistance to improve their health and wellbeing.

We offer a triage service where we will contact the referral, discuss their options and offer support, advice and encouragement to begin.

Who is eligible for referral

Live Life Well will provide activity and behavioural change to improve the health and wellbeing of referred clients at risk of or with chronic disease. The interventions are delivered by appropriately trained, insured and registered exercise instructors and the whole programme will be monitored to maintain quality assurance.

For the patient to be eligible for referral they must be inactive and meet at least one of the following criteria listed at a mild or moderate level:

  • Coronary heart disease risk factors
  • Musculoskeletal
  • Referral from a rehabilitation programme
  • Diabetes
  • Weight management
  • Osteoporosis
  • Respiratory problems
  • Mild anxiety/depression
  • Cancer

Please note the exclusion criteria below before making a referral:

  • Aged under 18
  • Cardio myopathy
  • Suspected or known aneurysm
  • Uncontrolled hypertension >180/100 (in either)
  • Uncontrolled resting tachycardia ≥ 100bpm
  • Uncontrolled diabetes
  • Uncontrolled / unstable angina
  • Uncontrolled epilepsy
  • Acute uncontrolled psychiatric illness
  • Unstable or acute heart failure
  • New or uncontrolled arrhythmias
  • Established coronary heart disease (including myocardial infarction)
  • Febrile illness
  • Experiences pain, dizziness or excessive breathlessness during exertion

How we can help

We have a range of activities from outdoor walking groups, online low level exercise as well as a range of classes within our facilities. From low level circuits classes, chair based classes, gentle exercise classes, yoga, pilates, aquafit and relaxation sessions our trained exercise referral specialists will ensure your patients are in safe hands.

We also offer a range of team sports across Aberdeenshire such as walking netball, walking football, table tennis and badminton. These sports can often make people feel more connected in their communities, increase confidence and reduce isolation.

Alongside the physical activities we offer, we also provide wellbeing services to help people become more sociable. These include conversation cafes, knit and natter and confidence to cook classes. It is our aim to enable everyone to feel connected where they live.
